Heather is one of Home Care Experts' Service Managers and one of the most experienced care coordinators on the team. With a background that spans years of direct support work, coordination, and service management in the disability sector, Heather brings a grounded, human-centred approach to every participant relationship she manages.
Her role sits at the intersection of clinical oversight and lived coordination. Heather manages a caseload of participants whose support needs range from moderate to highly complex, and she is responsible for ensuring that each one has a support plan that is accurate, well resourced, and genuinely reflective of who they are and what they want from their life.
Heather has a particular strength in building trust with participants and families who have had difficult experiences with previous providers. She takes the time to listen, to understand the full picture, and to build a plan that actually works in practice, not just on paper. Participants and families regularly describe her as the reason they stayed with HCE.
She works closely with the nursing team to ensure clinical requirements are integrated seamlessly into each support plan, and she is a consistent point of contact for support coordinators and allied health professionals who want to know their participant is in safe and capable hands.
